NDIS Support Coordination & Plan Management

How To Get NDIS Support Coordination or Plan Management
Step 1
First talk to your NDIS Planner Local Area Coordinator and discuss with them the level of Coordination Support you need in your NDIS plan.
Step 2
Once you have Support Coordination included as a part of your NDIS plan, you will be provided with a referral form from your NDIA Planner.
Step 3
Complete your form and send it to info@nationaldisabilitycentreaustralia.com.au to choose us as your NDIS Support Coordinator.
Step 4
Now you are all ready to get started with your Support Coordination service with National Disability Centre Australia. Our team will reach out to you to organise a time to meet and discuss your support needs.
